This condition makes times Down,s individuals Profoundly Retarded, are totally dependent on others and are usually institutionalized.

Main dental manifestations are gum periodontal infection, which is usually the result of dental neglect, Dental caries, Destructive oral habits; such as tongue thrusting, bruxism, clenching and drooling, big tongue, wrong bite, and teeth misalignment  and miseruption, delayed and small teeth

People with mental health problems are entitled to the same standards of care as the rest of the population, need regular mouth checks,  caregivers, and Parents’ Counseling to provide prevention.

Dental Profession’s Responsibility I do consider it is an obligation, and noble responsibility.

Recommendations

The General and oral health of these special group children have always been neglected, steps should be taken to provide basic medical and dental care facilities.

Dental facilities such as oral prophylaxis, periodic screening for dental caries, restoration for decayed teeth should be provided regularly. Special oral health programs to motivate and educate the children and their guardians should be carried out at regular intervals.

HELPING PATIENTS MAINTAIN EFFECTIVE SELF-CARE

the dexterity needed for effective brushing is impaired or nule, for Various grips can be added to toothbrushes to make them easier to use for patients who lack the dexterity needed for effective brushing.

Also many take medications that cause negative oral health side effects, such as dry mouth which increases caries

For patients who have difficulty grasping, the use of a wide-handled power toothbrush is recommended. The addition of bicycle grips, tennis balls, or soft plastic to toothbrush handles may also make them easier to hold

An extender (ruler, rod, or wooden spoon) can be taped to the handle of a toothbrush to provide assistance for patients with a limited range of motion

Caregivers should also receive patient education on the importance of oral health, the oral/systemic link, optimal nutrition, and oral hygiene techniques so they can help patients maintain their oral health

TREATMENT

Dental treatment need is high for people with disabilities requiring hospital dentistry treatment under General Anesthesia when cannot be expected any cooperation in regular basis dental chair. The main indication for treatment under GA is suspected or confirmed pain or dental complaints but many times cannot express  any pain due to their deep mental retardation

The caregivers have to be trained in nutrition control limiting the intake of cariogenic and erosive food, as well as in oral hygiene. The cooperation of caregivers and the dental team helps to avoid dental emergencies in patients who are difficult or very complex to treat.
